How To Give

Important notes:

• Payment method: Credit or debit card only.
• It may be easier to use a laptop, if you have one.

1. Go to the website:

Visit donate.trinityhealthathome.org

Your payment is secure and your information is never shared.

2. Choose your donation amount:

• Click one of the preset amounts: $25, $50, $100, $200
• Or click Other to enter a different amount.

3. Select where your gift goes:

• Find the dropdown menu labeled for agency selection.
• Scroll and select Georgia - St. Mary's Home Health & Hospice
• If you don’t have a preference on which agency to support, choose Trinity Health At Home National Organization (first option). This lets us use your gift where it’s needed most.

4. Optional: Dedicate your gift in honor or memory of someone:

• Use the dropdown to choose “In memory of” or “In honor of.”
• Enter their first and last name (optional).
• If you want us to notify their family, check the box and add their name, address or email.

5. Click “Add Donation.”:

This takes you to the next page.

6. Review your donation:

• You should see “Thank you for supporting Trinity Health At Home” at the top.
• Confirm your amount and agency selection.

7. Enter your personal information:

• Name, address, phone and email.
• If your billing address is different, uncheck the box and enter the correct billing address.

8. Click “Donate Now.”:

9. Enter payment details:

• Card number, expiration date and 3- or 4-digit security code (CSC).
• Click “Finish and Pay.”

10. You’re done!

• Your donation will appear on your card statement as HOMECARE HOSPICE LIVONIA USA
• You should be taken to a confirmation page and receive a confirmation email.

St. Mary's Home Health & Hospice 

2. Include a note with:

a. Your name, mailing address and contact info (so we can thank you).

b. If your gift is in honor or in memory of someone, include their name and any details needed to notify the family such as name and mailing address.
