Trinity Health At Home is nationally recognized as a leader in home-based care. Nearly all Trinity Health At Home agencies are accredited by The Joint Commission and meet or exceed quality metrics set by The Centers for Medicare and Medicaid Services (CMS).
Accreditations
The Joint Commission is the largest standards-setting and accrediting body in health care. Achieving this accreditation shows that we are committed to providing high quality services and improving patient care and safety.
Quality Achievements
The Centers for Medicare & Medicaid Services (CMS) is a federal agency created by the U.S. Department of Health and Human Services that provides health coverage to more than 160 million people. CMS partners with us and the entire health care community to improve quality, equity and outcomes in the health care industry.
CMS gives our agencies quality metric ratings, so we can improve in certain areas. Quality matters to us and that’s why we’re proud to present our most recent quality care data:
Home Health Care Quality Rating Highlights
- 14 of our agencies received 4 stars or more from The Centers for Medicare & Medicaid Services (CMS) for exceptional care quality.
- 12 of our agencies are exceeding the national average of “How often the home health team began their patients’ care in a timely manner” and are over 96.1%.
Hospice Care Quality Rating Highlights
- 5 of our agencies received a Hospice Index Score (1-10) of 9 or higher.
- 7 of our agencies are beating the national average of “Patients who got an assessment of all 7 HIS quality measures at the beginning of hospice care to meet the HIS Comprehensive Assessment Measure requirements” and are all over 91.6%.
