Caring for moms and babies at home: Our maternal-newborn program Our maternal-newborn home health nurses provide essential care and support to families during one of life’s most meaningful and vulnerable times. During a routine home visit, Natalie P., one of our newest nurses, noticed a newborn’s temperature was lower than expected. Trusting her instincts, she contacted the pediatrician, and the baby was referred to a local children’s hospital. That decision proved critical.
The baby was diagnosed with a rare but serious condition that required urgent medical care. Thanks to Natalie’s swift action, treatment began without delay. The baby’s parents later shared their deep gratitude for her attentiveness and compassion, calling her “an angel.”
Natalie is a team member for Tiny Blessings, the maternal-newborn division of Trinity Health At Home, serving families across the Philadelphia, Pennsylvania region. Each month, the team evaluates more than 700 moms and babies, helping ensure their health and safety through skilled care and early intervention.
Our maternal-newborn team supports families by:
- Monitoring newborn health and development
- Supporting new parents with education and resources
- Identifying early signs of complications
- Coordinating care with pediatricians and OB providers
- Offering emotional support during postpartum recovery
