Who We Are
Mount Carmel Home Care & Hospice is a member of Trinity Health At Home, a national home care and hospice care organization serving communities throughout eleven states. As a faith-based, not-for-profit, Mount Carmel Home Care & Hospice serves patients and families in the comfort of home, offering skilled nursing, therapy (physical, occupational, speech) and medical social work. We are Medicare certified and accredited by The Joint Commission.
Our Services
Home Care
We provide care to individuals recovering from illness or surgery and help manage chronic conditions. We are committed to enhancing the overall well-being of our patients. With our own virtual care program, patients benefit from 24/7 monitoring in real-time and are empowered to achieve health care goals and avoid preventable hospitalizations.
Hospice Care
We provide specialized end-of-life care and support to individuals with terminal illnesses, focusing on pain management, symptom relief, emotional comfort and spiritual guidance. Our interdisciplinary team of experts are committed to honoring the dignity and wishes of our patients while offering compassionate support to their loved ones.
Bereavement Support
Navigating the profound period of grief after losing a loved one requires understanding, support and community. We offer free bereavement support to anyone in our communities who has lost a loved one. Our specialized grief support groups address various aspects of the grief journey. Experienced bereavement counselors provide personalized sessions and our ongoing adult grief support group fosters a supportive environment. We also host community memorial events to honor and remember loved ones we’ve lost.

Our History
Mount Carmel Home Care & Hospice has a rich history of providing compassionate care to patients in the comfort of their own homes. We were established as part of the Mount Carmel Health System, which has a long-standing tradition of serving the community with a mission rooted in providing holistic and compassionate care.
Over the years, Mount Carmel Home Care & Hospice has expanded our services to meet the growing needs of patients requiring home-based medical care. This includes a wide range of services such as skilled nursing, physical therapy, occupational therapy, speech therapy and social work. We have continuously evolved by adding advanced medical technologies and practices to enhance patient care and help patients achieve goals.
