A Veteran’s Last Journey with Dignity and Love
When Sergio, a retired war veteran, entered hospice care with Trinity Health At Home, his care team went above and beyond to honor his life and service. Their efforts led to a powerful final chapter filled with comfort, dignity and celebration.

When Sergio, a retired war veteran, was diagnosed with a life-limiting illness, he began his end-of-life journey with Saint Agnes Home Health and Hospice, part of Trinity Health At Home, in Fresno, California. Like many patients receiving hospice care, Sergio found peace living his final days at home, surrounded by familiar comforts and compassionate support. But behind the scenes, his care team was planning something extraordinary.
Chaplain Steve E., a retired Army Colonel, Clinical Manager Carolyn W. and a group of dedicated volunteers reached out to the Central Valley Honor Flight association with a bold idea: to give Sergio one last meaningful journey. Their efforts came together just in time for Sergio’s 80th birthday, when he boarded a Veteran Honor Flight to Washington, D.C. Accompanied by fellow veterans and a guardian, Sergio visited the nation’s capital and its war memorials – places that held deep personal significance and honored his years of service.

Thanks to the hospice team’s unwavering dedication, Sergio’s final chapter was marked not only by comfort and care, but also by honor and celebration.
